  4. Copy of letter of identification from (LGA/State)
  5. Copy of Birth certificate/Attestation of Birth.
  6. Guarantor’s form sworn before the commissioner of Oaths/Magistrate/High Court Judge
  7. Guarantor’s recent colour passport photograph
  8. Copy of guarantors e-passport date page.
  9. Cash Payment of PLN170 administrative visa fee, to be made to the Embassy’s Account Section by each applicant, after which a receipt will be issued.

  9. Cash Payment of PLN170 administrative visa fee, to be made to the Embassy’s Account Section by each applicant, after which a receipt will be issued.
