Consular & Education

Nigerian nationals:

  1.  Application Letter/email addressed to the Embassy.
  2.  Coloured copy of Passport Data and Current Visa pages.
  3.  Coloured copy of Residence Card (front and back).
  4.  Two coloured Passport Size Photographs.
  5.  Birth Certificate from National Population Commission (NPC) or Statutory Declaration of Age from High Court in Nigeria with a barcode for verification;
  6.  Affidavit from High Court in Nigeria with a barcode for verification, declaring the single status of Applicant (bachelorhood/spinsterhood) or divorce certificate where applicable.

Non-Nigerian spouse:

  1.   Birth Certificate and translated copy in English.
  2.  Coloured copy of Identity Card (front and back).
  3.  Two coloured Passport Size Photographs.
  4.  Statement of Marital Status from relevant Polish authorities (for Polish nationals) and Statement of Marital Status legalized at respective Foreign Ministry and Embassy/High Commission (for non-Polish nationals).
  5.  Intending couple is to physically appear for an interview at a time determined by the Embassy.

NOTE:

  1. Cash Payment of PLN360 is to be made to the Embassy’s Account Section for each Nigerian applicant, after which a receipt will be issued and presented to the Consular Section; and
  2. Marriage documents Issued by the Embassy are valid for a period of three (3) months from the date of issuance.
  1.  Application Letter/email addressed to the Embassy.
  2.  Coloured copy of Passport Data and Current Visa Page.
  3.  Birth Certificate from National Population Commission (NPC) or Statutory Declaration of Age from High Court in Nigeria with a barcode for verification.
  4.  Birth Certificate and translated copy in English (for non-Nigerian nationals).
  5.  Coloured copy of Student Identity Card;
  6.  Coloured copy of O-level result (front and back) authenticated at the Federal Ministries of Education and Foreign Affairs; and
  7.  Valid Scratch Card Pin and Serial Number.

NOTE:

Cash Payment of PLN240 (and PLN120 per additional document) is to be made to the Embassy’s Account Section for each applicant, after which a receipt will be issued and presented to the Consular Section.

*Nigerians are encouraged to apply to only Government schools and Accredited schools*

  1.  Application Letter/email addressed to the Embassy;
  2.  Coloured copy of Passport Data and Current Visa Page;
  3.  Birth Certificate from National Population Commission (NPC) or Statutory Declaration of Age from High Court in Nigeria with a barcode for verification;
  4.  Birth Certificate and translated copy in English (for non-Nigerian nationals); and
  5.  Valid Drivers Licence.

NOTE:

  1.  Letter from the appointed undertaker addressed to the Head of Mission (Chargée d’affaires a.i. or Ambassador).
  2.  Authorization letter by the deceased’s family member (Phone No., Full Address in Nigeria, Undertaker appointed).
  3.  Copy of family representative Identity Card (International passport data page/Voters card/Drivers license/National ID card).
  4.  Deceased death certificate.
  5.  Copy of Deceased passport data page.
  6.  Waiver for Repatriation of Human Remains provided by the Ministry of Health in Abuja, Nigeria.
